FALL 2012 / SPRING 2013 SEASON SIGN-UP INFORMATION

Registration for the upcoming Fall 2012/Spring 2013 Season will be as follows:
Players must be between the ages of 4 - 18 years old as of July 31, 2012 to be eligible to play.  For U5 teams, the players need to be 4 by September 1, 2012 to be eligible to play.  

Cost is $55 per player before June 1, 2012.  The fee will increase to $70 after June 1 and $85 after July 1.  Anyone registered after this date will not be guaranteed a spot on a team and may be placed on a waiting list.

The registration fee includes 6 games in the fall and 6 games in the spring, team jersey, shorts, socks, and secondary insurance.  Fees are also used for field equipment and maintenance, coach supplies, as well as coach, board member, and referee training.

For U-14, and U-16/19 players, the registration fee is $35.  Players will receive a jersey to use for the season which will be turned in after each season to keep the costs down for these age groups.

You may pre-register on eayso after April 1, 2012 for the Fall 2012/Spring 2013 season.  Pre-register online @ www.eayso.org .  Our region is 1350.  Simply sign on for a user name and password and follow the instructions for registering a player.  You must fill out an application for each player that you are registering.  Online pre-registration provides a convenient, time-saving process to register players and volunteers for participating in AYSO.  There will be a "fast lane" at all registration events for those who have pre-registered.  Bring 2 copies of your signed form, birth certificate for any new players to our region, and volunteers must bring a valid driver's license or state ID. (This is to show proof of residency.  We only need to look at this information, not keep a copy of it.)  You are not registered until the signed forms are turned in with payment to one of our three registration events or mailed to the Registrar.

Print, sign and date both forms (send a copy of your child's birth certificate, if have not previously played for Hopkins AYSO) and attend a registration event or
